Wet aanpak meervoudige problematiek sociaal domein

Gemeenten, zorg- en hulpverleners, uitvoeringsorganisaties en cliënten in het sociaal domein krijgen nieuwe taken, bevoegdheden en waarborgen rond samenwerking en gegevensverwerking.

• College onderzoekt meervoudige problematiek op verzoek van cliënt of betrokken professional.
• Gecoördineerde aanpak krijgt coördinator, casusoverleg en regels voor afstemming tussen partijen.
• Gegevensverwerking krijgt grondslagen, inclusief beperkte verwerking van gezondheids- en strafrechtelijke gegevens.
• Gemeenten moeten een meldmogelijkheid bieden voor zorgen over welzijn, gezondheid, zelfredzaamheid of participatie.

Inwerkingtreding

De wet treedt in werking op een bij koninklijk besluit te bepalen tijdstip, eventueel verschillend per artikel of onderdeel.

Samenhang met

Er is samenloop geregeld met de Wet gegevensverwerking door samenwerkingsverbanden, Kamerstuk 35 447, voor afstemming met Zorg- en Veiligheidshuizen.
